High blood pressure is defined as 140 mmHg over 90 mmHg, and for years, doctors have used that measure as the threshold for prescribing anti-hypertensive drugs. But based on new recommendations, adults who are 60 or older can wait until their readings reach 150 over 90 or above to begin medication. After reviewing available evidence on the effects of blood pressure treatments, the Institute of Medicine’s Eighth Joint National Committee (JNC 8) concluded that aggressive treatment can lead to lightheadedness, falls and fainting in elderly populations so they advised loosening the guidelines for starting medication.

But days after the guidelines were published, heart experts raised concerns about how the advice would affect patients; inadequately controlled blood pressure is a risk factor for heart attack and stroke.

Members of the JNC8 who voted against the recommendations published an editorial in the Annals of Internal Medicine outlining their reasons for opposing the change. “We, the panel minority, believed that evidence was insufficient to increase the target goal from its current level because increasing the goal may cause harm by increasing the risk for CVD(心血管疾。゛nd partially undoing the remarkable progress in reducing cardiovascular mortality in Americans older than 60,” they wrote.

Dr. Suzanne Steinbaum said she has not changed the way she treats her patients over 60. She says, “As a preventive cardiologist(心臟病學(xué)家), these new guidelines have made me crazy. What we have learned is that blood pressure treatment even for a patient above 80 has been shown to be critical. It goes against everything we know as cardiologists.”

Defending the panel’s decision, Dr. Paul A. James, co-chairman of the guidelines committee, said in an email to TIME, “I can assure you that the panel discussed the opinions of the minority members on three different occasions and the majority were not persuaded that the expert skills of a few members should override the scientific evidence.”

James said that as with any medications, doctors will use the blood pressure recommendations as guidelines, and prescribe drugs based on their evaluation of each patient. “Medications prescribed by physicians all have the potential to be dangerous,” he wrote. “Educating doctors about the scientific evidence will help doctors make better decisions with patients. Physicians who practice on the basis of scientific evidence usually subscribe to simple rule if the medicine cannot be shown to help the patient, then the medicine should not be prescribed.”

The editorial isn’t likely to change them any time soon. But the concerns should urge doctors to monitor their patients more closely and tailor medications and their doses more carefully.

Many old people work well into their 70s and 80s, running families, countries or corporations. Other people,however, despite being fit and highly talented, are forced to retire in their fifties or even earlier because of the regulations of a company or the nation. This essay will examine whether people should be allowed to continue working as long as they want or whether they should be encouraged to retire at a particular stage.

Some people think there are several arguments for allowing older people to continue working as long as they are able. First of all, older employees have an immense amount of knowledge and experience which can be lost to a business or organization if they are made to retire. A second point is that older employees are often extremely loyal employees and are more willing to carry out company policies than younger less committed staff. However, a more important point is regarding the attitudes in society to old people. To force someone to resign or retire at 60 indicates that the society does not value the input of these people and that effectively their useful life is over. Age is irrelevant to a working life, surely if older employees are told they cannot work after 60, this is age discrimination. That they become old does not necessarily mean they are going to be sick. Old people could be more aware, experienced and committed than some youngsters.

Others, however, think that allowing older people to work indefinitely is not a good policy. Age alone is no guarantee of ability. Old people are only ambitious workaholics who are too selfish and self-centered to believe that a younger person could do better. Actually, many younger employees have more experience or skills than older staff, who may have been stuck in one area or unit for most of their working lives. Having compulsory retirement allows new ideas in an organization. In addition, without age limits, however, many people would continue to work purely because they did not have any other plans or roles. A third point of view is that older people should be rewarded by society for their life’s labor by being given generous pensions and the freedom to enjoy their leisure. We now have youngsters who can’t find jobs because old people are choosing not to retire. Old people are not retiring because this new generation of “old people” think they will never die due to modern advances in medicine.

With many young people unemployed or frustrated in low-level positions, there are often calls to compulsorily retire older workers. However, this can affect the older individual’s freedom and right to work and can deprive(剝奪) society of valuable experience and insights. I feel that giving workers more flexibility and choice over their retirement age will benefit society and the individual.

The Tower of London, not a single tower but a large group, was built to guard London.

Besides William and Henry VIII, the tower’s history has other famous names in British history. In 1389 a clerk of works (現(xiàn)場(chǎng)監(jiān)工員) named Geoffrey Chaucer — author of The Canterbury Tales — oversaw (監(jiān)督) the building of the Tower Wharf. Today the jewels remain in the tower as they have since Blood’s day.

For six centuries this place was also home to a group of animals from elephants to big cats. It stopped being used as an animals’ home in 1835 and its animals were moved to the London Zoo.

Today visitors can walk the walls, visit guard towers and see the crown jewels. Guards at the tower, popularly known as beefeaters, not only guard the tower but also give interesting tours that are among the most popular parts of any visit.

How to Get There

Trains and the Dockland Light Railway stop near the tower. Buses 15, 42, 78 and 100 reach the tower, and riverboats stop at Tower Pier. Taxis, bicycles, and foot power are also good ways to reach the tower. Driving is a less attractive choice because of the expensive parking.

When to Go

The tower is open all year round, except during the Christmas holidays (December 24 to 26) and January 1. School holidays and summertime are the busiest times.

How to Visit

As expected at a place that has seen so much history, there is enough of interest at the Tower of London to keep visitors busy for days. Stop at the Welcome Center for visitor information on everything from the crown jewels to family fun activities.
